Remove the pre-selected itemset when adding itemset to an exam

There is a risk for error if the pre-selected item set is incorrect and then overlooked. We find it is rarely correct anyways.
The ability to search for an itemset more than just by id number would be helpful. After multiple years of administering exams, the item set list will get lengthy.
